Bronze plaque attached to cylindrical fountain structure featuring text and portrait relief of Tetrazzini. Completed by artist Haig Patigian.

The memorial is located in San Francisco, CA. It was erected in honor of Tetrazzini, for her open-air concert at the location of Lotta's Fountain on Christmas eve of 1910. The official unveiling of the plaque occurred on March 24th of 1912.
