Small copper square plaque with rounded top embedded in rock. Embossed text surrounding circular emblem in center.

The grave and marker are located together at the Rock Ridge Cemetery in Sharon, MA. Both her birth name and her brother's name, which she enlisted in the war under, are recorded on the grave.

The plaque was placed there by the Deborah Sampson Chapter of the Daughters of the American Revolution, marking Sampson's site of burial and honoring her time as a soldier.
